Asking for a raise

I am a CMA and have been in the medical field for 9 years. I first started off as a CNA. I became a CMA in 2007. I have been working for this internal medicine physician for 2 years and only make 9.50 an hour. I did some research on the pay for medical assistants and they should be making 12.00-13.00 an hour. How do I go about asking for a raise without upsetting my employer or making her feel that I am unsatisfied? Someone please help me ask for a raise.
